HealthPoints Staff

Having managed the Care Management and Disease Management division of UnitedHealth Group, (with responsibility for 55 million lives it is the largest operation of its type in the US), HealthPoints’ executive team has the experience to effectively and efficiently run large population health programs. Through work at UnitedHealth as well as other related job experiences, staff are leaders in healthcare operations, call center operations, technology integration and management, disease management, public health management, management consulting and clinical diabetes management.


Through the experience at UHG, HealthPoints staff came to understand that the deficiency of UHG’s business, and that of all of its traditional Disease Management competitors, was the lack of current, actionable data. HealthPoints was born out of the realization that success in improving the lives and health of our patients was heavily dependent upon gathering, analyzing and taking action on real-time biometric data. Additionally, when combined with data from sources such as claims systems and electronic medical records, HealthPoints can demonstrate not only improved population health outcomes, but also document Return On Investment (ROI) for its clients. HealthPoints’ proprietary technologies support the data collection, analysis and presentation necessary to successfully manage chronically ill populations. HealthPoints’ Medical Information Intelligence technology and rich patient data supports research that requires the ability to “slice and dice” various biometric and pharmacological data.
