Many clients may already have some form of disease or care management services in place already, though these may not include the full range of integrated programs provided by
HealthPoints.

In order to ensure the HealthPoints does not provide unnecessary duplicative services (which would drive unnecessary added costs) — as well as to ensure that existing services and resources are still being optimally used — HealthPoints can flexibly adjust its programs to meet a client’s unique needs.

Key steps provided by HealthPoints include:

• Evaluation: Current care services are assessed to identify existing strengths as well as opportunities
  
• Recommendation: Specific HealthPoints services that may benefit the patient base are prioritized
