Program Execution — Care Management and Discharge Planning

Care management refers to the process of managing a patient’s care (i.e., ensure they’re receiving only the necessary services) during acute times of high, costly health care utilization. The most common example is when an individual is admitted to a hospital. In this case, care management focuses on pre-admission counseling, ensuring timely discharges, and minimizing readmission rates.
