Another potential high cost population of patients are those at high risk of, or in early phases of developing, chronic illnesses. Using its predictive modeling algorithms, HealthPoints can identify these individuals for placement in HealthPoints’ Structured Monitoring and Wellness Programs.

Once identified for placement in the program:

HealthPoints develops individualized monitoring plans. For example, for a pre-diabetic this would include periodic blood glucose, HbA1c, cholesterol and blood pressure screenings.
HealthPoints would provide reminders to the patients to ensure screenings are performed.
HealthPoints provides education and information to the patients — customized to the patient’s unique needs.
